As a key member of our integrated Care Team, you'll work alongside a Registered Nurse and Social Worker to provide exceptional care to patients with complex medical needs. Your Responsibilities:
Conduct comprehensive annual wellness exams in patients' homes and virtually.
Educate and counsel patients and families about available resources and programs.
Maintain accurate and up-to-date patient records, including medical history, diagnoses, and treatment plans.
Coordinate care with primary care providers, specialists, and ancillary services.
Complete all documentation and paperwork promptly.
Deliver evidence-based, timely care to reduce hospitalizations and improve patient satisfaction.
Prescribe medications, order tests, and collaborate with physicians.
Facilitate closing gaps in care through patient education and diagnostic testing coordination.
Assist patients with accessing educational resources.
Participate in integrated care team meetings.
Develop and implement patient-centered treatment plans.
Follow up and evaluate patient progress.
Perform other assigned duties.
What You'll Bring:
Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
Board certification (NP: ANCC/AANP; PA: NCCPA).
Current and unrestricted DEA certificate.
Ability to work autonomously and without direct supervision.
Access to transportation, a valid driver's license, and car insurance.
Proficiency with medical instruments, equipment, and computer-based data management systems.
Excellent verbal and written communication skills.
Knowledge of HIPAA and OSHA regulations.
2+ years of direct patient care experience.
Managed Care/IPA/Health Plan experience.
Experience conducting annual wellness visits or similar comprehensive visits virtually or in the home.
Bilingual (English/Spanish) highly preferred.
Basic Life Support (BLS) certification.
